Objectives To conduct an independent evaluation of the first phase of the Health Foundation’s Safer Patients Initiative (SPI), and to identify the net additional effect of SPI and any differences in changes in participating and non-participating NHS hospitals. Design Mixed method evaluation involving five substudies, before and after design. Setting NHS hospitals in the United Kingdom. Participants Four hospitals (one in each country in the UK) participating in the first phase of the SPI (SPI1); 18 control hospitals. Intervention The SPI1 (...) was a compound (multi-component) organisational intervention delivered over 18 months that focused on improving the reliability of specific frontline care processes in designated clinical specialties and promoting organisational and cultural change. Results Senior staff members were knowledgeable and enthusiastic about SPI1. There was a small (0.08 points on a 5 point scale) but significant (P<0.01) effect in favour of the SPI1 hospitals in one of 11 dimensions of the staff questionnaire (organisational climate). Qualitative evidence showed only modest penetration of SPI1 at medical ward level. Although SPI1 was designed to engage staff from the bottom up, it did not usually feel like this to those working on the wards, and questions about legitimacy of some aspects of SPI1 were raised. Of the five components to identify patients at risk of deterioration—monitoring of vital signs (14 items); routine tests (three items); evidence based standards specific to certain diseases (three items); prescribing errors (multiple items from the British National Formulary); and medical history taking (11 items)—there was little net difference between control and SPI1 hospitals, except in relation to quality of monitoring of acute medical patients, which improved on average over time across all hospitals. Recording of respiratory rate increased to a greater degree in SPI1 than in control hospitals; in the second six hours after admission recording increased from 40% (93) to 69% (165) in control hospitals and from 37% (141) to 78% (296) in SPI1 hospitals (odds ratio for “difference in difference” 2.1, 99% confidence interval 1.0 to 4.3; P=0.008). Use of a formal scoring system for patients with pneumonia also increased over time (from 2% (102) to 23% (111) in control hospitals and from 2% (170) to 9% (189) in SPI1 hospitals), which favoured controls and was not significant (0.3, 0.02 to 3.4; P=0.173). There were no improvements in the proportion of prescription errors and no effects that could be attributed to SPI1 in non-targeted generic areas (such as enhanced safety culture). On some measures, the lack of effect could be because compliance was already high at baseline (such as use of steroids in over 85% of cases where indicated), but even when there was more room for improvement (such as in quality of medical history taking), there was no significant additional net effect of SPI1. There were no changes over time or between control and SPI1 hospitals in errors or rates of adverse events in patients in medical wards. Mortality increased from 11% (27) to 16% (39) among controls and decreased from 17% (63) to 13% (49) among SPI1 hospitals, but the risk adjusted difference was not significant (0.5, 0.2 to 1.4; P=0.085). Poor care was a contributing factor in four of the 178 deaths identified by review of case notes. The survey of patients showed no significant differences apart from an increase in perception of cleanliness in favour of SPI1 hospitals. Conclusions The introduction of SPI1 was associated with improvements in one of the types of clinical process studied (monitoring of vital signs) and one measure of staff perceptions of organisational climate. There was no additional effect of SPI1 on other targeted issues nor on other measures of generic organisational strengthening. (shrink)

We give several characterizations of Schnorr trivial sets, including a new lowness notion for Schnorr triviality based on truth-table reducibility. These characterizations allow us to see not only that some natural classes of sets, including maximal sets, are composed entirely of Schnorr trivials, but also that the Schnorr trivial sets form an ideal in the truth-table degrees but not the weak truth-table degrees. This answers a question of Downey, Griffiths and LaForte.

We study arithmetic and hyperarithmetic degrees of categoricity. We extend a result of E. Fokina, I. Kalimullin, and R. Miller to show that for every computable ordinal $\alpha$, $\mathbf{0}^{}$ is the degree of categoricity of some computable structure $\mathcal{A}$. We show additionally that for $\alpha$ a computable successor ordinal, every degree $2$-c.e. in and above $\mathbf{0}^{}$ is a degree of categoricity. We further prove that every degree of categoricity is hyperarithmetic and show that the index set of structures with degrees (...) of categoricity is $\Pi_{1}^{1}$-complete. (shrink)

We study the connection between Schnorr triviality and genericity. We show that while no 2-generic is Turing equivalent to a Schnorr trivial and no 1-generic is tt-equivalent to a Schnorr trivial, there is a 1-generic that is Turing equivalent to a Schnorr trivial. However, every such 1-generic must be high. As a corollary, we prove that not all K-trivials are Schnorr trivial. We also use these techniques to extend a previous result and show that the bases of cones of Schnorr (...) trivial Turing degrees are precisely those whose jumps are at least 0". (shrink)

We show that van Lambalgen's Theorem fails with respect to recursive randomness and Schnorr randomness for some real in every high degree and provide a full characterization of the Turing degrees for which van Lambalgen's Theorem can fail with respect to Kurtz randomness. However, we also show that there is a recursively random real that is not Martin-Löf random for which van Lambalgen's Theorem holds with respect to recursive randomness.

We show that every countable ideal of degrees that are low for isomorphism is contained in a principal ideal of degrees that are low for isomorphism by adapting an exact pair construction. We further show that within the hyperimmune free degrees, lowness for isomorphism is entirely independent of computable traceability.

The weakly random reals contain not only the Schnorr random reals as a subclass but also the weakly 1-generic reals and therefore the n -generic reals for every n . While the class of Schnorr random reals does not overlap with any of these classes of generic reals, their degrees may. In this paper, we describe the extent to which this is possible for the Turing, weak truth-table, and truth-table degrees and then extend our analysis to the Schnorr random and (...) hyperimmune reals. We investigate the relationship between lowness for Schnorr randomness and Schnorr triviality. We show that a real is low for Schnorr randomness if and only if it is Schnorr trivial and hyperimmune free.

We introduce several highness notions on degrees related to the problem of computing isomorphisms between structures, provided that isomorphisms exist. We consider variants along axes of uniformity, inclusion of negative information, and several other problems related to computing isomorphisms. These other problems include Scott analysis (in the form of back-and-forth relations), jump hierarchies, and computing descending sequences in linear orders.

A real is Martin-Löf (Schnorr) random if it does not belong to any effectively presented null ${\Sigma^0_1}$ (recursive) class of reals. Although these randomness notions are very closely related, the set of Turing degrees containing reals that are K-trivial has very different properties from the set of Turing degrees that are Schnorr trivial. Nies proved in (Adv Math 197(1):274–305, 2005) that all K-trivial reals are low. In this paper, we prove that if ${{\bf h'} \geq_T {\bf 0''}}$ , then h (...) contains a Schnorr trivial real. Since this concept appears to separate computational complexity from computational strength, it suggests that Schnorr trivial reals should be considered in a structure other than the Turing degrees. En el ámbito de la lógica matemática existe un problema sobre la relación lógica entre dos versiones débiles del Axioma de elección (AE) que no se ha podido resolver desde el año 2000 (aproximadamente). Tales versiones están relacionadas con ultrafiltros no principales y con Propiedades Ramsey (Bernstein, Polarizada, Subretículo, Ramsey, Ordinales flotantes, etc). La primera versión débil del AE es la siguiente (A): “Existen ultrafiltros no principales sobre el conjunto de los números naturales (ℕ)”. Y la segunda versión débil del (...) AE es la siguiente (B): “Existen ultraflitros sobre ℕ”. Se sabe que A implica B, pero se desconoce si B implica A. Di Prisco y Henle conjeturan en los artículos ([1], [2]) que esto no ocurre, es decir, conjeturan que B no implica A, en otras palabras, conjeturan que A es más fuerte estrictamente que B, que A es independiente de B, pero esto no se ha podido demostrar todavía aunque se ha intentado hacer desde hace aproximadamente 21 años. Ultrafilters are very important mathematical objects in mathematical research [6, 22, 23]. There are a wide variety of classical theorems in various branches of mathematics where ultrafilters are applied in their proof, and other classical theorems that deal directly with ultrafilters. The objective of this article is to contribute (in a divulgative way) to ultrafilter research by describing the demonstrations of some such theorems related (uniquely or in combination) to topology, Measure Theory, algebra, combinatorial infinite, set theory and first-order logic, (...) also formulating some updated open problems of set theory that refer to non-principal ultrafilters on N, the Mathias’s model and the Solovay’s model. -/- Resumen: Los ultrafiltros son objetos matemáticos muy importantes en la investigación matemática [6, 22, 23]. Existen una gran variedad de teoremas clásicos en diversas ramas de la matemática donde se aplican ultrafiltros en su demostración, y otros teoremas clásicos que tratan directamente sobre ultrafiltros. The objective of this article is to present an original proof of the following theorem: Thereis a generic extension of the Solovay’s model L(R) where there is a linear order of P(N)/fin that extends to the partial order (P(N)/f in), ≤*). Linear orders of P(N)/fin are important because, among other reasons, they allow constructing non-measurable sets, moreover they are applied in Ramsey's Theory .

Es conocido que el método de forcing es una de las técnicas de construcción de modelos más importantes de la Teoría de conjuntos en la actualidad, siendo el mismo muy útil para investigar problemas de matemática y/o de fundamentos de la matemática. El destacado matemático Joan Bagaria afirma lo siguiente sobre el método de forcing en su artículo "Paul Cohen y la técnica del forcing" (Gaceta de la Real Sociedad Matemática Española, Vol. 2, Nº 3, 1999, págs 543-553) : "Aunque (...) Cohen recibió la medalla Fields por su demostración de la independencia de la hipótesis del continuo y del axioma de elección, su contribución va mucho más allá de estos problemas. Su nuevo método, el forcing, no sólo ha permitido resolver un sinfín de problemas importantes en prácticamente todas las áreas de las matemáticas, si no que ha cambiado para siempre nuestra concepción de la matemática como ciencia". El objetivo principal del siguiente trabajo es estudiar el método de forcing describiendo algunas de sus aplicaciones (forcing de Cohen, forcing aleatorio, forcing de Mathias, forcing de Sacks, forcing de Silver, etc.), y ofreciendo una aproximación a sus fundamentos metamatemáticos. We give a relativistic treatment to the dynamics of spherical bodies rotating at very high speed. It is found that most of the mass of a homogeneous spherical quark with Franklin rotation is due to the relativistic increase of the mass.

ABSTRACTAdults perceive emotional expressions categorically, with discrimination being faster and more accurate between expressions from different emotion categories than between two stimuli from the same category. The current study sought to test whether facial expressions of happiness and fear are perceived categorically by pre-verbal infants, using a new stimulus set that was shown to yield categorical perception in adult observers. These stimuli were then used with 7-month-old infants using a habituation and visual preference paradigm. Each year since 1983 the American Council of Learned Societies has invited one of America's leading scholars to deliver the Haskins Lecture, in honor of Charles Homer Haskins, a distinguished scholar and teacher who was instrumental in the founding of the ACLS. In this volume, which commemorates the 75th anniversary of the ACLS, Douglas Greenberg and Stanley Katz bring together the lectures presented by ten of America's most distinguished scholars. Each lecture is a personal and intellectual glimpse into the "life (...) of learning" of such leading scholars as Maynard Mack, Annemarie Schimmel, and John Hope Franklin. The lectures focus on self-reflection of lives dedicated to learning, rather than on scholarship in the usual sense of the term. Ranging from being forced to learn Latin to painful memories of war and racism, the lecturers all recount stories from their eventful lives. Each offers thoughts on the body of work he or she has produced and the forces, personal and intellectual, that have shaped it. ABSTRACT Rosalind Franklin is best known for her informative X-ray diffraction patterns of DNA that provided vital clues for James Watson and Francis Crick's double-stranded helical model. Her scientific career did not end when she left the DNA work at King's College, however. In 1953 Franklin moved to J. D. Bernal's crystallography laboratory at Birkbeck College, where she shifted her focus to the three-dimensional structure of viruses, obtaining diffraction patterns of Tobacco mosaic virus (TMV) of unprecedented detail and (...) clarity. During the next five years, while making significant headway on the structural determination of TMV, Franklin maintained an active correspondence with both Watson and Crick, who were also studying aspects of virus structure. Developments in TMV research during the 1950s illustrate the connections in the emerging field of molecular biology between structural studies of nucleic acids and of proteins and viruses. They also reveal how the protagonists of the “race for the double helix” continued to interact personally and professionally during the years when Watson and Crick's model for the double-helical structure of DNA was debated and confirmed. Prediction-based accounts of language acquisition have the potential to explain several different effects in child language acquisition and adult language processing. However, evidence regarding the developmental predictions of such accounts is mixed. Here, we consider several predictions of these accounts in two large-scale developmental studies of syntactic priming of the English dative alternation. Study 1 was a cross-sectional study (N = 140) of children aged 3−9 years, in which we found strong evidence of abstract priming and the lexical boost, but (...) little evidence that either effect was moderated by age. We found weak evidence for a prime surprisal effect; however, exploratory analyses revealed a protracted developmental trajectory for verb-structure biases, providing an explanation as for why prime surprisal effects are more elusive in developmental populations. In a longitudinal study (N = 102) of children in tightly controlled age bands at 42, 48, and 54 months, we found priming effects emerged on trials with verb overlap early but did not observe clear evidence of priming on trials without verb overlap until 54 months. There was no evidence of a prime surprisal effect at any time point and none of the effects were moderated by age. The results relating to the emergence of the abstract priming and lexical boost effects are consistent with prediction-based models, while the absence of age-related effects appears to reflect the structure-specific challenges the dative presents to English-acquiring children. Overall, our complex pattern of findings demonstrates the value of developmental data sets in testing psycholinguistic theory. (shrink)

Bu yazıda Rosalind Franklin örneği üzerinden kadın bilimcilerin bilim tarihinde nasıl ele alındığını tartıştım. Kadınlara bilim tarihinde hak ettikleri yeri verme çabamız sırasında bilim tarihçileri olarak nelere dikkat etmemiz gerektiğini ele aldım. Özellikle vurguladığım noktalar bilimsel araştırma biçimlerinin doğru yansıtılması, kurumsal ayrımcılıkların üstünün örtülmesinden kaçınılması ve ele alınan figürün bilime katkısının (...) açıkça yazılması oldu. Yazıda Franklin hakkındaki literatüre bir katkı sunmayı amaçladım ancak daha da önemlisi Franklin başta olmak üzere kadın bilimciler hakkındaki metinlerde sık yapılan hatalara işaret etmek istedim. -/- Following the realization that history cannot be written objectively, there have been some transformations in the historiography of science. Although these changes are generally positive, some historiography debates have not been adequately reflected in historical texts on science. In this article, I discussed how women scientists are represented in the history of science through the case of Rosalind Franklin. In our effort to give women the place they deserve in the history of science, I have discussed what should we pay attention to as historians of science. My particular emphasis was on reflecting the structure of scientific research correctly, avoiding covering institutional discrimination and clearly writing the contribution of the figure discussed to science. I intend to make a contribution to the literature on Franklin, but more importantly, I tried to point out some common mistakes in texts about women scientists, especially Franklin. (shrink)

The use of brain imaging technology as a common tool of research has spawned concern and debate over how investigators should respond to incidental fndings discovered in the course of research. In this article, we argue that investigators have an obligation to respond to incidental fndings in view of their entering into a professional relationship with research participants in which they are granted privileged access to private information with potential relevance to participants' health. We discuss the scope and limits of (...) this professional obligation to respond to incidental fndings, bearing in mind that the relationship between investigators and research participants differs fundamentally from the doctor-patient relationship. Cosmetic surgery is a fast-growing medical practice. In 1997 surgeons in the United States performed the four most common cosmetic procedures443,728 times, an increase of 150% over the comparable total for 1992. Estimated total expenditures for cosmetic surgery range from $1 to $2 billion. As managed care cuts into physicians' income and autonomy, cosmetic surgery, which is not covered by health insurance, offers a financially attractive medical specialty.

Morals and politics depend on a metaphysical backing. All reality is marked by certain necessary features and a divine purpose inherent in all reality defines the good to which all human life should be directed. These are bold assertions in a climate where the credibility of metaphysics is widely denied. Indeed, for the past two centuries, Western philosophy has been marked by a consensus that questions about moral and political life should be considered separately from questions about ultimate reality. In (...) this challenging work, Franklin I. Gamwell defends metaphysical necessity against both modern and postmodern critiques. The metaphysics vindicated is not the traditional form both critiques typically have in view, however. Instead, Gamwell outlines a neoclassical project for which Alfred North Whitehead and Charles Hartshorne are the main philosophical resources. As it maintains the significance of theistic metaphysics, the book makes no appeal to religious authority but solely to common human experience, and on this basis articulates principles of human purpose and democratic justice.
